Transition

Sometime between the ages of 18 and 21, people with CF will transition to the adult CF center at Shands. People with CF, their families and the Pediatric Pulmonary Team will decide together the best time for this change. The move to adulthood and adult care is an exciting one. The person with CF is no longer a child but is embarking on an adult path and is in charge of his/her own care. The change involves taking on more responsibility, in school, work, finances, recreation and health care. Towards this end, the PPC has developed and/or acquired some checklists to help people with CF think about the skills they will need, or the questions they should ask, as they move towards adulthood.
